Enrollment
Those
eligible for the Medical Access Program must:
• live in Baraga, Gogebic, Houghton or Keweenaw County,
• be 19 to 64 years old,
• have no medical coverage of any kind nor access to
affordable medical coverage,
• and have a total household income of 200 percent
of the Federal Poverty level or less.
Before
eligible patients are seen through the Medical
Access Program, they are interviewed to be sure they
do not qualify for benefits through other programs,
such as Medicare, Medicaid, Veterans’ benefits or
other medical programs. Those who do not qualify for
any other assistance are approved for enrollment in
WUPHAC's Medical Access Program and assigned a
doctor within a week after the enrollment interview.
Every effort will be made to assign enrollees to a
doctor with whom they have an existing relationship,
if that doctor participates in the program.
During your enrollment, we will :
• Confirm your proof of residency, income, and age
• Complete WUPHAC’s Medical Access Program
application
• Complete a Medicaid application if one has not been
submitted in the last 90 days
• Review a form listing your responsibilities under the
program
• Review an information release so WUPHAC can share
necessary information with medical providers and
the Family Independence Agency
• Collect information about your prescription
medication needs
• Review the Medical Access Program benefits and
coverage, including the pharmacy program
• Assist you in applying for financial assistance from
Portage Health System or Keweenaw Memorial
Medical Center if extensive medical services not
covered under our program are anticipated
• Make referrals to other agencies if necessary
Within a week to ten days after your enrollment you
will receive confirmation of your approval for the
program. You will also receive your assigned doctor’s
name and phone number, information on making
appointments for services, a detailed list of services
covered and not covered, and a PAC card for use at your assigned physicians office and local pharmcy.
What to bring:
• Proof of identification
• Proof of residency
• Monthly income information or most recent tax return if requested
• Medicaid denial letter if one has been received in
the last 90 days
• A list of your prescription medications, including
dosages
